What It Is:
This is an ABC Data Sheet. It is a worksheet with a table containing columns for 'Date,' 'Time,' 'Antecedent,' 'Behavior,' and 'Consequence.' The 'Antecedent' column prompts for a description of what, where, who, and how right before the behavior. The 'Behavior' column prompts for a description of what behaviors occurred, their intensity, and duration. The 'Consequence' column prompts for a description of what immediately followed the behavior, what changed in the environment, and what others' reactions were. The table has multiple rows to record multiple observations.
