Information
Description
What It Is:
This is a student behavior contract worksheet for Corona Avenue School. It includes spaces for the student's name, a specific goal, and a chart to track behavior during different parts of the school day: from A.M. to Recess, Recess to Lunch, and Lunch to Recess. The chart allows for tracking behavior over multiple days. An 'O' in the chart represents a reward. The worksheet also has a space for a parent's signature. There are two contracts on one page.
Grade Level Suitability:
This worksheet is suitable for grades 1-6. The simple format and focus on behavior tracking make it accessible to younger students, while the concept of setting goals and monitoring behavior is also relevant for older elementary students.
Why Use It:
This worksheet helps students take ownership of their behavior by setting a specific goal and tracking their progress throughout the day. It provides a visual representation of their behavior and encourages positive reinforcement through rewards. It also promotes communication between the student, teacher, and parent regarding behavior expectations and progress.
How to Use It:
First, the student and teacher should collaboratively define a specific, measurable, achievable, relevant, and time-bound (SMART) goal and write it on the line provided. Then, the teacher will observe the student's behavior during the specified time periods (A.M. to Recess, Recess to Lunch, Lunch to Recess) and mark an 'O' in the corresponding box if the student meets the behavioral expectations. At the end of the day or week, the student and teacher can review the chart, and if the student has earned enough 'O's, they receive a reward. The parent then signs the contract to acknowledge their awareness of the student's behavior and progress.
Target Users:
This worksheet is designed for elementary school students who need support in managing their behavior. It is also useful for teachers who want to implement a behavior management system in their classroom and for parents who want to be actively involved in their child's behavioral development.
