What It Is:
This is an Insulin/Carbohydrate Ratio Log Sheet from Unity Diabetes Center. It has columns for the date, pre-meal blood glucose (BG), grams of carbs, and units of insulin for each meal.

Before bedtime, record your blood glucose level (Bedtime BG). Repeat this process daily to track your insulin and carbohydrate ratios.
